Please note that the application form (below right) is a fill-in form.  If you do not see the active fill-in fields, scroll down below the document viewing frame below and select to download the document so that you can save, re-open, and then fill in the document from storage on your device (the downloaded version will display the fill-in fields).  Also, once the form is filled in, the signatures may be added either by printing, signing, and then scanning or by saving to PDF and using the free Adobe Reader signature tool or by using the using the free Docusign Add In available in Word in the Insert menu, My  Add-ins.
